Title Band
If you want to print a title on first page
of the form, you need to add a 'Title Band' element onto the
form. Press the 
button and select the 'Title' band as the band type. You can
insert the label elements with 'FORMachine', 'Common Name', 'Species
Name', 'Category' & 'Photo' onto the header band.
